The present case demonstrates that the uropathogenic E. hermanii clone can cause destruction of the kidneys. During asymptomatic bacteriuria or cystitis, the bacteria remain in the urinary tract. Even when pyelonephritis develops, inflammatory response of the host is still restricted to the urinary tract. These signs mean that uropathogenic E. hermanii may be not very virulent.

This study has evaluated an enrichment and four subculture procedures for detection of Escherichia coli O157 in raw meat products. The combination of enrichment in modified tryptone broth incubated at 42 degrees C for 6 h, followed by immunomagnetic separation and subculture on to cefixime, tellurite sorbitol MacConkey agar was the most sensitive and selective procedure. Traditional subculture using 10 microliters and 100 microliters inocula and culture of centrifuged deposits were less satisfactory. A most probable number method was used to enumerate E. coli O157 in naturally contaminated samples associated with human cases. The results indicated that the samples contained < 0.3 to 2300 cfu g-1 of E. coli O157 which confirms that the infective dose for this organism is low.

Antibiotic stewardship is universally agreed to be desirable, but optimal models for stewardship remain uncertain. UK stewardship targets the particular antibiotic families-cephalosporins and fluoroquinolones-blamed for the selection of Clostridium-difficile-associated disease. To balance this there have been dramatic increases in the use of penicillin-β-lactamase inhibitor combinations. By channelling selection pressure in this way, we hazard destroying the utility of these antibiotic classes in turn, as happened with gonorrhoea where penicillins, fluoroquinolones and cefixime were sequentially lost as therapies. Strikingly, in context, almost all carbapenemase-producers are highly resistant to penicillin-β-lactamase inhibitor combinations, which may select for them. There is an urgent need to explore an alternative stewardship model, seeking to limit total antibiotic use but to maintain heterogeneity in what is used, avoiding concentrated selection pressure. There is also a great need to improve and accelerate diagnostics for infection and resistance, reducing or removing the need for protracted empirical treatment with broad-spectrum agents.

There is continued interest in the development of oral beta-lactam compounds, which can be used clinically to treat various bacterial infections, particularly those caused by beta-haemolytic streptococci. Cefixime is a new orally active cephalosporin, with a broad spectrum of antibacterial activity, including Enterobacteriaceae, Haemophilus influenzae, Branhamella catarrhalis, Streptococcus pneumoniae and Streptococcus pyogenes. Cefixime is highly resistant to hydrolysis by most beta-lactamases. In this study the authors examined the effects of this molecule on Group A and Group B beta-haemolytic streptococci, recently isolated from clinical specimens in the authors' laboratory. MICs and the growth curves of 36 strains of Group A streptococci and the effects of sub-MICs on buccal cell adhesion were evaluated. The results show that concerning the sub-MIC cefixime effect on streptococci adherence, the treatment led to a decrease in adherence to the cells of the strains studied. Moreover cefixime showed good activity with 86.1% of the strains with MIC less than or equal to 0.5 microgram/ml, and the growth curves demonstrated that the molecule possesses a bactericidal effect after 3 h. Concerning Group B streptococci, 70.3% of the strains showed a MIC less than or equal to 2 micrograms/ml. In conclusion cefixime demonstrates good activity on beta-haemolytic streptococci, particularly those of Group A.

The in vitro activity of BAY v 3522 was compared with the activities of cephalexin, cefaclor, cefuroxime, cefixime, and amoxicillin-clavulanate. MICs (in micrograms/ml) of BAY v 3522 were as follows: Staphylococcus spp. (except for oxacillin-resistant strains), 0.13 to 1; Streptococcus spp. (except for some viridans group streptococci), less than or equal to 0.015 to 0.25; Enterococcus faecalis, 2 to 8; other enterococci, 0.5 to greater than 32; beta-lactamase-negative Haemophilus influenzae and Branhamella catarrhalis, 0.13 to 1; beta-lactamase-positive H. influenzae and B. catarrhalis, 0.5 to 4; Pasteurella multocida, 0.06 to 0.25; and members of the family Enterobacteriaceae, 0.5 to greater than 32. Among the cephalosporins, BAY v 3522 was the most active against gram-positive cocci and cefixime was the most active against gram-negative bacilli; BAY v 3522 was similar in activity to amoxicillin-clavulanate against most species.
Overall, 855 (41%) patients had either S. Typhi (n=581,28%) or S. Paratyphi A (n=274,13%) cultured from blood; 1,237 (59%) were culture negative. There were 139 (6.6%) treatment failures with one death. Except for the last trial with ceftriaxone, the fluoroquinolone gatifloxacin was associated with equivalent or better fever clearance times and lower treatment failure rates in comparison to all other antimicrobials. However, we additionally found the minimum inhibitory concentrations (MIC) against fluoroquinolones have risen significantly since 2005 and were associated with increasing fever clearance times. Notably, all organisms were susceptible to ceftriaxone throughout the study period (2005-2014) and the MICs against azithromycin declined, confirming the utility of these alternative drugs for enteric fever treatment.
The application of pharmacokinetic (PK) and pharmacodynamic (PD) data in conjunction with minimum inhibitory concentrations (MICs) of antibacterial agents has been shown to allow for improved selection and appropriate dosing of antimicrobial agents for specific infections, increasing the likelihood of bacteriologic cure and, through this, reducing the risk for the development of resistant organisms.
